Your instructor needs the form below prior to your first class session.

Here are instructions to submit your completed form:

1.  Click the double arrow (>>) at the right top of the image below.
2. Choose the Download option and save the file to your Desktop (or to a location easy for you to find).
3. Open the file with Adobe Reader
4. Right-click the open file and save with a new name
5. Open the newly named file; complete and save the file
6. Email the saved file to gmchenry@uams.edu
